The Evergreen Assisted Living Residence is a seven-year-old facility located in the rural community of O'Neill, Nebraska. The Evergreen is located on the northwest edge of town, on US Highway 281. The 22,900 -square-foot facility is one level, containing 33 apartments and 40 licensed beds.
